Jamie Flesher is a Registered Nurse and a Certified Geriatric Care Manager with over 30 years of experience in the health care field. She obtained her Nursing Degree at Harper College and additional study and certification in elder care management from the University of Florida. Her years of experience working in the hospital and home health, has afforded her the opportunity to work directly with the elder population and individuals with disabilities. This has provided her an understanding of the enormous challenges faced by individuals during the aging process especially those experiencing age-related illnesses.
Jamie has been the CEO of Legacy Home Care, LLC since 2006 and has achieved the Care Manager Certified (CMC) designation through the National Association of Certified Care Managers and Certified Managed Care Nurse through the American Association of Managed Care Nurses. She is a member of the National Association of Professional Geriatric Care Managers, a member of the Arizona Non-Medical Home Care Association, serves as a Co-Chair for the East Valley Committee on Aging, and founder of the Alliance Care Team. She is committed in helping seniors and the chronically disabled remain independent in their homes for as long as possible by providing quality non-medical home care services and geriatric care management. She uses her years of experience and continued education in the area of elder care to advocate for her clients to ensure that they are receiving the best care possible during hospitalizations and assist with the transition back to home or assisted living arrangement.
She is an experienced guide and resource for families of seniors and those with chronic health problems. Her focus is always patient centered as she conducts assessments and develops a care plan to identify problems and provide solutions. She will screen, arrange, and monitor in-home medical and non-medical services. In addition, she will accompany clients to doctor's appointments and communicate with family members and healthcare professionals. When needed, she provides family caregiver training and support to enable family members to work toward providing their loved one with the care and dignity they deserve. Our mission is to ensure a better quality of life for our elderly clients and their families by providing dependable and affordable care as we leave a legacy of caring from the heart.
She looks forward to working with the team of IHMS of AZ, LLC and the transitional programs
